Applies the No Surprises Act patient-provider dispute rule, 45 CFR 149.620 and 149.610: eligible when a provider's billed charges are at least $400 over its estimate, 120 calendar days from the initial bill to dispute, no collection placement or threats and suspended late fees while a dispute is pending, settlement reduction of at least half the administrative fee, and collecting no more than the SDR-determined amount.
